Health and Lifestyle consultation FAQs
The Carers Grants Project provides financial support to unpaid carers for essential items or services that improve wellbeing and make caring easier. Grants are funded by local and national charitable organisations.
How often can I have a Health & Lifestyle Consultation (HLC)?
You can have a HLC every year.
How many sessions are included?
An initial one-hour appointment, plus up to three follow-up sessions over the next three months (if you would like them).
Where do the HLCs take place?
Sessions can take place in person, over the phone or on Zoom.
During the first session, you can have some physical health checks (detailed below). If you’d like to have these checks, you will need to come in-person.
The first session can still go ahead over the phone or on Zoom if you would prefer not to have these physical checks.
What physical health checks are included?
We can measure your blood pressure, body composition, height and weight. All these checks are optional.
When are appointments available?
Appointments are currently offered Tuesday to Friday, 9am–5pm, with some flexibility.
Do I need a professional referral or pre-assessment?
No. You can refer yourself directly by calling us on 0207 4288950 or by email at referrals@camdencarers.org.uk. Bringing any useful information to your appointment is helpful but not required.
Will I need to undress?
No, you don’t need to undress for any of the checks.
You will need bare feet to use the body composition machine so we wouldn’t recommend wearing tights to your appointment.
What types of questions will be asked?
We will ask a range of different lifestyle questions. These will cover nutrition, exercise, smoking, alcohol, mental health, sleep, NHS screening programmes and reminders about services you’re entitled to.
All these questions are optional; you don’t have to answer anything you are not comfortable with.
What will happen in my follow-up appointment?
Yes:
-
We cannot fund items that have already been purchased.
-
Do not buy the item before your grant outcome is confirmed, as we cannot reimburse costs.
Do I have to do all parts of the HLC?
No, you can choose which areas to focus on.
Are HLC appointments confidential?
Yes, all discussions will stay between you and the Health Lead.
If you would like us to share your information with an external service (e.g. a stop smoking service), we can do this with your consent. We won’t make any referrals without your consent.
However, if we believe that you are at risk, any concerns will need to be raised with senior management, as per our safeguarding policy.
Can former carers have an HLC?
Yes, former carers are entitled to 13-months of support after their caring role ends.
Will you support me to access other services?
Yes, we can signpost or refer you to onward support if needed.
